Personal Care or Home Health Aide
The HELPsource™ Home Health Aide is responsible for providing personal care services, companionship, escort, and light housekeeping services to clients in their home. Home care is provided according to the care plan prepared by the registered nurse and carried out under the supervision of the RN. The care plan is developed with care managers, physicians and or long term care insurance providers.
RESPONIBILITIES:
- Assist client in maintaining good personal hygiene including; bathing services, shower, tub, bed baths, mouth, skin and hair care
- Assist with the use of toilet facilities including bedpans
- Assist with transfers, utilizing mechanical equipment, i.e. Hoyer Lifts when applicable
- Assist with walking, including assistance with walkers, canes and wheel chairs
- Medication reminders
- Record fluid intake and output and document on appropriate forms
- Assist Physical and or Occupational Therapy personnel with range of motion exercises after proper instruction
- Promote client’s mental alertness by engaging in appropriate dialogue or reading of newspapers, books or magazines with client
- Doing crossword and jigsaw puzzles to promote memory skills
- Planning and preparing nutritious meals
- Assist with special diets and feeding
- Cleans up the cooking and eating area following each meal
- Keeps bathroom & bedroom areas clean and free of safety hazards
- Performs light housekeeping tasks
- Laundry duties / Bed Linen changes as needed
- Establishes a relationship with client based on trust and confidentiality
- Notifies the RN and/or scheduling coordinator if a patient has increased care needs
- Reports any changes in client’s mental or physical condition to the RN and/or scheduling coordinator
- Demonstrates proper universal precautions and infection control techniques
- Maintains a caring and positive attitude
- May provide Escort services to client’s Doctor Appointments, Grocery shopping (with or without the client) or to Family events as requested
QUALIFICATIONS:
- A compassionate nurturing disposition
- Certificate of completion of an approved Nurse Aide / Home Health Aid Training Program (Minimum 77-hours)
- A passing grade in a Written Skills evaluation
- A passing grade in a Clinical Skills demonstration
- Completion of Orientation and Mentoring
- A High School graduate or GED
